Parents wishing for their child(ren) to be baptized at St. John or St. Michael Church need to fill out the form below to attend a baptismal preparation class. At least one parent needs to attend the St. John baptismal preparation class. The preparation class takes place on the 3rd Monday of each month at 7:30pm in our chapel. If you have had a child baptized at St. John before, and have attended the prep class for a child, you do not need to attend it again for additional children. Scheduling the baptism through the office is the only requirement. Those wishing to have a baptism performed at St. Gabriel need to complete this printable form and return it to the St. Gabriel Parish Office.
